CFI Case Manager (Southeast and Seacoast NH)
Crotched Mountain Community Care is currently hiring for a Choices for Independence (CFI) Case Manager for New Hampshire's Southeast and Seacoast region. CMCC Case Managers, provide independent integrated case management to New Hampshire seniors and adults with chronic illnesses and/or disabilities, substance use disorder, and behavioral health diagnosis in accordance with agency policy, applicable state regulations and accepted professional standards of practice.
Work is performed with minimal supervision and a high degree of independence in keeping with the standards of excellence set forth by the program. CMCC Case Managers utilize their home office and are required to travel for monthly client in-home/in-person visits. Ideal candidates will be based within a 90 minute drive from their caseload. This forty hour position is benefit eligible, includes generous paid time off and a $2,500 sign on bonus to be distributed over the first year of employment. Starting salary is contingent on experience.

.QUALIFICATIONS:
- Have a degree in a human-services related field and one year of supervised experience OR hold valid RN license with two years relevant experience
- Have demonstrated knowledge of the local service delivery system and the resources available to participants
- Have demonstrated knowledge of the development and provision of integrated, person-centered services
SCHEDULE:
- 40 hours Monday through Friday
- Home office based with monthly travel for in person client visits
PRIMARY COMPETENCIES:
- Conduct comprehensive assessments of client’s condition and psycho-social needs, including health literacy status and deficits, and develop a person centered integrated case management plan collaboratively with the client and family or caregiver
- Conduct monthly client contacts in compliance with state regulations for the purpose of monitoring the effectiveness of the care plan and client goals, make changes to care plan as indicated for client safety and stability.
- Facilitate communication and coordination between members of the health care team, involving the client and family/guardian in the decision-making process in order to minimize fragmentation in the services
- Educate the client, the family or caregiver, and members of the health care delivery team about community resources, insurance benefits, psycho-social concerns, case management, etc., so that timely and informed decisions can be made
- Empower the client to problem-solve by exploring service options, and alternative plans, when necessary, to achieve desired outcomes
- Encourage the appropriate use of health care services and strive to improve quality of care and maintain cost effectiveness on a case-by-case basis
- Assist the client in the safe transitioning of care to the next most appropriate level
- Strive to promote client self-advocacy and self-determination
- Advocate for both the client to facilitate positive outcomes for the client, in a safe and cost effective manner
- Participate in CMCC Shared Governance Committees and community meetings and organizations as appropriate to the position or assigned by the supervisor
- Improve client safety and satisfaction
- Represent CMCC in a professional and courteous manner when interacting with clients, families, community provider agencies and DHHS representatives
- Maintain required documentation consistent with agency policy and state regulations.
