What is Crotched Mountain Community Care (CMCC) Case Management?
Case Managers are professionals who serve as client advocates supporting, guiding, and coordinating care for clients, families, and caregivers as they navigate their health and wellness journey.
All CMCC case managers have a range of education and experience in fields like Social Work, Mental Health, Nursing, Addiction, and Elder Care.
For over 35 years CMCC has been committed to helping New Hampshire residents remain independent, safe, and healthy in their own homes.


Supporting People Throughout New Hampshire
CMCC provides comprehensive case management services for individuals on the New Hampshire CFI Medicaid waiver program. CMCC Case Managers coordinate with provider agencies to provide in-home services. Some examples include; assistance with personal care, light housekeeping, grocery shopping, transportation, nursing, home modifications, and medical equipment.
CMCC Case Managers help clients navigate the health care system by advocating and assisting with applications, and necessary documentation to maintain Medicaid and CFI coverage, housing resources, heating and electric assistance, and residential care resources.
What to Expect
Professionals from our case management team meet with you in your home to discuss your individual needs. We will work with you and your existing support network (family, friends, mental health professionals, health care provider) to create a person-centered service plan. Based on this plan, CMCC case managers then coordinate services provided by local agencies, government programs, or community organizations.
CMCC Case Managers will continue to regularly monitor your service plan by phone calls and visiting you in your home bi-monthly, making changes to your plan and services as needed or desired.
Thanks to our affiliation with Crotched Mountain Foundation, CMCC has grant funding available, which can offer clients additional resources including medical equipment and home modifications not covered by insurance as well as items needed for basic needs.
