The case managers of Crotched Mountain Community Care (CMCC) have had to adapt to support the nearly 1,000 individuals they serve throughout New Hampshire during COVID-19 – but that hasn’t slowed their impact on the lives of their clients.
In just October, CMCC case managers:
- Performed over 2,000 well-check calls to adults with chronic medical needs and disabilities throughout New Hampshire
- Helped Linda move into an accessible apartment so she could remain in the community
- Assisted Tom who was on the cusp of being evicted secure funding to stay in their apartment
- Helped a Richard obtain a necessary communication device
In addition, six clients, working with CMCC case managers, received approval for durable medical equipment and home modifications, enabling them to remain safely in their homes. And CMCC secured grant funding for groceries for several clients as well as funding for needed dental services.
