A Shared Home and a Life of Maximum Independence

Individuals are matched with a certified Home Care Provider and live with them in their home while receiving the support they need to thrive. Each person’s program is individualized to fit their unique needs. Participants are supported to maintain their existing social network of family and friends as well as build new connections in the community.

Experiencing a Life in the Community Together

Enhanced Family Care and Community Participation Services are individualized utilizing person centered planning. CMCC’s Adult Services team works collaboratively with clients, their families, guardians, service coordinators and health care providers to support the individual in all areas of their life, including their needs, goals, and health.

Our goal is to support those we serve to live their most fulfilling life in their own community.

We are a New Hampshire Medicaid provider who serves people through the Developmentally Disabled (DD) and Acquired Brain Disorder (ABD) Medicaid waiver programs. To receive our services, contact your service coordination agency who can begin the referral process with you.

Crotched Mountain offers Enhanced Family Care programs with our without Community Participation Services for adults with developmental and intellectual disabilities or acquired brain disorders.
