Join our team! Crotched Mountain Community Care (CMCC) is currently hiring for a Choices for Independence (CFI) Case Manager for New Hampshire’s Seacoast region including but not limited to the towns of Somersworth, Farmington, Rochester and Barrington.
CMCC Case Managers provide independent integrated case management to New Hampshire seniors and adults with chronic illnesses and/or disabilities, substance use disorder, and behavioral health diagnosis in accordance with agency policy, applicable state regulations and accepted professional standards of practice.
